To prepare for your DOT Physical:
- Continue taking your prescribed medications as usual.
- Drink water before your appointment. You will need to give a urine sample upon arrival.
- Do not consume high sugar foods the day of your exam. Eating a lot of sugar can cause glucose to excrete into your urine.
What do I need to bring to a DOT physical?
- Valid driver's license and/or other photo ID
- List of medications, including strength and dosage for both prescription and over the counter
- Eye glasses or contact lenses, if applicable. If you recently had an eye exam, it would be helpful, but not required, to bring that report
- Working hearing aids, if applicable
- Are you diabetic? If yes, you MUST bring an A1C report less than or equal to 6 months old
- Do you take insulin for your diabetes? If yes, you must have your medical doctor complete form MSCA-5870 and bring it to your appointment.
- Medical doctor(s) contact information, including name(s), phone and fax numbers
- Current FMCSA Waiver or Exemption Certificate, or Skills Performance Evaluation, if applicable
